Reporting to the Team Manager, the post holder works closely with Social Workers and Duty Workers to support older people and adults with physical disabilities. The primary purpose of this role is to assist in the review and reassessment of individual care plans, ensuring that services are effectively monitored and that service users receive practical advice and support. The role demands a commitment to the Council's values of being Open, Fair, and Together to deliver the best outcomes for the people of Cardiff. Responsibilities: Arrange and monitor appropriate services in strict alignment with individual care plans. Empower disabled and elderly people and their carers throughout the assessment and review process. Seek to understand others and treat all individuals with respect, as per the Behavioural Competency Framework. Undertake all duties in compliance with Council Policies, Procedures, and Health and Safety legislation. Participate in training and self-development, while contributing to the development of others in the team. Qualifications: Educated to GCSE standard or equivalent Essentials: Proven experience working with older people and adults with physical disabilities within a professional environment. A deep understanding of the specific needs of elderly and disabled individuals. A full valid driving licence with sole access to a vehicle during working hours.
