Responsibilities
The Hospice Aide, under the supervision of a registered nurse, provides personal direct patient care services as outlined in the Plan of Care.
* Reports patient condition and environment to the Executive Director or designee after each encounter, following the Care Management process.
* Assists with activities of daily living (ADLs) such as mobility, transfers, walking, grooming, bathing, dressing, eating, and toileting during each patient visit.
* Performs household services necessary to support the patient's health and prevent or delay institutionalization.
* Participates in QAPI initiatives by identifying issues and reporting to the administrator as needed.
* Provides emotional support and compassionate care to patients and families regularly.
Education and Experience
License Requirements
* Successful completion of a competency evaluation prior to providing patient care.
* Current CPR certification.
* Valid driver's license, vehicle insurance, and access to reliable transportation.
Additional State-Specific Requirements
* In Alabama, certification is preferred.
Skill Requirements
* Ability to handle the demands of a hospice aide position maturely and effectively.
* Ability to read, write, and follow directions accurately and promptly.
