What You’ll Do
You’ll be at the heart of our “Home First” approach, assessing patients in their own homes to prevent avoidable hospital admissions and support safe, timely discharge from hospital — aligned with the Scottish Government’s Discharge Without Delay initiative. As a member of the multidisciplinary team you will build trusted relationships, work collaboratively with the service Moray wide to deliver care that truly puts people first.
What You’ll Bring
5. NMC registration as a Specialist Practitioner District Nurse (2nd Year SPDN students will also be considered). If you're nearing qualification, we’ll support your transition into the role and offer Annex 21 pay until you’re fully registered.
6. A passion for person centred care and holistic assessment
7. Confidence in advance clinical assessment/clinical decision-making and compassionate leadership
8. Strong communication skills and a commitment to listening deeply
9. A growth mindset — always looking for ways to improve care
10. A full, current drivers licence and access to a car is essential.
