We are very excited to be recruiting a nurse associate for the Burnley West PCN. A knowledge of PCNs and what we are trying to achieve is essential for this role.
Role Responsibilities:
* Deliver evidence-based, high-quality, patient-centred, compassionate care consistently.
* Be an accountable professional.
* Work as part of a designated clinical and care team, focusing on the direct needs of the individual.
* Operate independently at times without direct supervision.
* Deliver care independently within the individual’s care plan and scope of practice.
* Access clinical and care advice when needed.
Key Responsibilities:
* Participate in the planning and delivery of the Enhanced Health in Care Homes model of proactive care for residents, families, and staff.
* Conduct Chronic Disease Reviews with housebound patients.
* Engage in clinical sessions within a practice setting.
