The role:

We are looking for a skilled and motivated Occupational Therapist to join our Rapid Assessment Link (RAL) service within the East Dunbartonshire Community Rehabilitation Team. The post holder will deliver specialist multidisciplinary assessment, case coordination, and Occupational Therapy intervention for adults with complex needs. You will work at the interface of acute and primary care, supporting early discharge, preventing avoidable hospital admissions, responding to direct referrals to RAL, and contributing to the Hospital at Home service. This role is central to ensuring timely, person‑centred rehabilitation and reablement within the community.

Duration, Location, and Working Pattern:

Full-time, permanent

Part‑time hours may be considered

Base location: Kirkintilloch Health and Care Centre, East Dunbartonshire Community Rehabilitation Team

Shift pattern, TBC

Key responsibilities:

* Multidisciplinary assessment and case coordination.

* Occupational Therapy assessment, treatment, and review.

* Supporting planned discharge for adults with complex needs.

* Working across acute and primary care to facilitate early discharge and prevent avoidable admissions.

* Responding directly to referrals to RAL.

* Providing OT input to the Hospital at Home service.

* Planning, coordinating, delivering, and evaluating the OT service within the Rehabilitation Service.

* Liaising with targeted areas within Acute and Hospital at Home to support discharge pathways.

Knowledge, training, qualifications and/or experience required to do the job:

* Degree in Occupational Therapy or equivalent.

* HCPC registration (essential).

* Driver’s licence (essential).

* Post‑graduate experience.

* Proven clinical expertise across a wide range of areas, ideally in both acute and community settings.
