Support Worker

This is for a support worker in a mental health Supported Living service for 15 adults. We provide 24/7 support which includes waking night staff.
Shifts include working weekends and doing waking nights per rota'd shifts.
The role is to support people who live in the home to live the life they choose without limits, with support to identify their hopes, dreams and goals and be supported to go on their journey to achieve these things in their life. On a daily basis the team will support individuals with Daily living activities such as medication, appointments, accessing community facilities, activities, outings/holidays.
Clients will work with you to produce and update their support plans so everyone follows this how they want and you will help clients to identify their risks, supporting them with positive risk taking but ensuring there are safeguards in place.
You will be expected to complete daily notes for each client that are meaningful and reflect the clients' goals, choices and preferences. You will also be expected to have a good understanding of Health and Safety and maintain the service to good standards in all areas, this includes cleaning.

The role is to support people who live in the home with daily life skills and activities, Updating support plans and risk assessments. Updating daily notes and ordering medication and general housing keeping of the service.
Working with other professionals involved in the clients care.
You will also be administering medication and supporting clients to health care appointments.

Detailed job description and main responsibilities
Your role will involve working with other professionals involved in the clients care and, in some instances, advocating for the client so their needs are clearly communicated so appropriate resources can be put in place to support the client on their life journey and that this is reflected in the support plan.
Your role involves supporting clients with their medications, you will need to be fully trained to our standards and be signed off as competent to carry out this task. Once competent you will be expected to understand what medication each client has, what they are for and who and how to seek appropriate support.
You will support clients to take their own medication through observation and verbal prompting, in some cases, administering the medication.
