Belmont Medical Centre is a friendly, inclusive, patient-focused GP surgery in South Hereford with a patient population of around 10,400. We are part of WBC Primary Care Network Services Ltd, collaborating with GPs, nurses, HCAs, clinical pharmacists and a strong admin team.

Job Details

Location: Belmont Medical Centre, Eastholme Avenue, Belmont, Hereford, HR2 7XT

Salary: £46,000 to £55,000 per year pro rata, dependent on experience

Contract: Permanent

Working pattern: Full-time or Part-time

Job Description / Responsibilities:

* Deliver a high standard of patient care as a Nurse Practitioner in general practice, using advanced autonomous clinical skills.
* Manage a clinical caseload, handling presenting patients’ needs in a primary care setting.
* Telephone triage and seeing patients; review and action laboratory results; sign prescriptions (if independent prescriber).
* Support urgent clinical queries; mix of face-to-face and telephone consultations; manage minor illnesses, acute presentations and long-term conditions within scope.
* Work with the clinical team; participate in regular team meetings; access CPD opportunities and further training.


Key Responsibilities

* Manage patients with acute and chronic medical conditions; provide advice and undertake same-day urgent triage.
* Identify red flags and underlying pathology; take appropriate action.
* Use complex decision-making for diagnosis, investigation and management within scope.
* Develop and evaluate treatment plans for chronic disease patients.
* Identify, manage and support patients at risk of long-term conditions; prevent adverse health effects.
* Provide advanced, specialist nursing care in line with evidence, NICE and NSF guidelines.
* Assess and triage patients; provide definitive treatment or refer to other team members as appropriate.
* Process and interpret pathology results; maintain accurate clinical records compliant with regulatory standards.
* Ensure SNOMED CT coding is used effectively; maintain chronic disease registers; chaperone patients when needed.
* Support patients in using prescribed medicines or OTC medicines within scope; review annually as required.
* Contribute to practice targets (QOF, PCN targets, IIF) following local guidelines.
* Complete relevant training to support multi-professional practice and CPD supervision.
* Liaise with external services to support vulnerable patients; delegate clinical responsibilities safely.
* Support safeguarding matters per local and national policies.

Knowledge, Skills & Qualifications

* Essential: Registered General Nurse (NMC); recognised NP qualification at degree level or equivalent.
* Understanding of primary care policy, GMS contract, Clinical Governance, NICE guidelines and national standards; awareness of medico-legal aspects of the NP role; commitment to equality and diversity.
* Skills: risk assessment and safe management, IT skills, strong interpersonal and communication skills, time management, data analysis, and ability to collaborate with the organisation, CCG and external stakeholders.
* Experience: minimum 5 years post-registration; evidence of CPD; experience with undifferentiated presenting problems; autonomous and team-based work; prescribing experience within current legislation; experience in chronic disease management (COPD, asthma, diabetes, CHD, AF).
* Desirable: experience with medical software, strategic thinking, presenting to wider audiences; interest or qualification in women’s health.
