We are looking to employ an experienced practice nurse tojoin our friendly team at Filey Surgery. The post will largely centre aroundthe management of long-term conditions and as such it is essential that perspectivecandidates have appropriate qualifications and experience in the management ofdiabetes and respiratory conditions. We are a training practice and encouragedevelopment of all our staff and we are committed to staff wellbeing and providing a supportive workplace.
The working pattern for the role will be 22- 28 hours over 3 - 4 days.
Main duties of the job
The practice nurse is tasked with delivering nursingservices and works as a member of the practice's multidisciplinary team,providing care within their scope to the designated patient population.Responsibilities include various clinical areas including health promotion,effective management of patients with long term conditions, preventativehealth, wound management and any other primary care nursing including in ourcontract.

Job responsibilities
The following are the core responsibilities of the practicenurse. There may be on occasion a requirement to carry out other tasks; thiswill be dependent on factors such as workload and staffing levels:
a. Develop, implement and embed health promotion andwell-being programmes
b. Implement and evaluate individual treatment plans forchronic disease patients
c. Identify, manage and support patients at risk ofdeveloping long-term conditions, preventing adverse effects on patients health
d. Provide routine nursing care to patients as required inaccordance with clinical-based evidence, NICE and the NSF
e. Provide wound care (ulcer/Doppler etc.) to patients
g. Request pathology services as necessary
h. Process pathology results as required
i. Provide chronic disease clinics, delivering patient careas necessary, referring patients to secondary/specialist care as required
j. Maintain accurate clinical records in conjunction withextant legislation
k. Ensure read codes are used effectively
l. Maintain chronic disease registers
m. Chaperone patients where necessary
n. Assist GPs with minor surgery when required
o. Prioritise health issues and intervene appropriately
p. Support the team in dealing with clinical emergencies
q. Recognise, assess and refer patients presenting withmental health needs
r. Implement vaccination programmes for adults and children
s. Support patients in the use of their prescribed medicinesor over-the-counter medicines (within own scope of practice)
t. Liaise with external services/agencies to ensure thepatient is supported appropriately (vulnerable patients etc.)
u. Delegate clinical responsibilities appropriately(ensuring safe practice and that the task is within the scope of practice ofthe individual)
v. Support the clinical team with all safeguarding matters,in accordance with local and national policies
w. Understand practice and local policies for substanceabuse and addictive behaviour, referring patients appropriately
x. Deliver opportunistic health promotion where appropriate
Secondary responsibilities
In addition to the primary responsibilities, the practicenurse may be requested to:
a. Engage in audit of services as and when required
b. Monitor and ensure the safe storage, rotation anddisposal of medicines
c. Support junior members of the nursing team, providingguidance when necessary
d. Participate in local initiatives to enhance servicedelivery and patient care
e. Support and participate in shared learning within thepractice
f. Continually review clinical practices, responding tonational policies and initiatives where appropriate
g. Participate in the review of significant and near-missevents applying a structured approach, i.e. root cause analysis (RCA).

* Experience of infection prevention and control measures

Qualifications
* Registered Nursing Qualification (RGN)
* Qualification in Immunisation and Vaccination Administration
* Qualification in Cervical Cytology (Smear Taker)
* Qualification in Diabetes and/or Respiratory Disease Management
* Mentor or Teaching Qualification Post Graduate Diploma or Degree (Chronic Disease Management)
* Nurse Prescriber Qualification
