We are seeking a compassionate and dedicated Palliative Care Social Worker to join our multidisciplinary team at St Luke’s Hospice. The successful candidate will serve as an expert resource in social work for palliative care, providing essential support and guidance to patients, family members, and caregivers. This role encompasses a wide range of responsibilities, contributing to the holistic well-being of individuals facing life-limiting illnesses and their families.

Key Responsibilities:
Communication and Relationships:
* Collaborate effectively within a multidisciplinary team, participating in meetings and handovers to ensure comprehensive care.
* Manage a diverse caseload independently, prioritizing specialist care needs.
* Assess individual and family strengths, resources, and coping skills to facilitate effective interventions.
* Conduct direct supportive work with patients, families, and caregivers, utilizing various techniques and involving external agencies when necessary.
Knowledge, Training, and Experience:
* Stay informed about local and national resources relevant to palliative care decision-making.
* Participate in educational programs to enhance palliative care knowledge from a social work perspective.
* Utilize advanced assessment skills to plan and implement appropriate interventions promptly.
Planning and Organization:
* Contribute to the ongoing development of St Luke’s services, advocating for the role of social work in strategic planning.
* Manage workload effectively within organizational policies, exercising professional judgment and best practices.
* Assess and manage risk while ensuring the safety and well-being of patients and caregivers.
Policy and Service Development:
* Ensure clinical practice aligns with national and local standards, protocols, and procedures.
* Lead or participate in service improvement projects, utilizing recognized tools for enhancement.
* Engage in practice-based audit, evaluation, and research as required.
Qualifications and Skills:
* Degree in Social Work (or equivalent qualification).
* Registered with the Health and Care Professions Council (HCPC).
* Demonstrated experience in palliative care or a related field.
* Excellent communication and interpersonal skills.
* Ability to work autonomously and as part of a team.
* Strong organizational and analytical abilities.
