About The Role
As a Support Worker, you will be at the forefront of providing high-quality, enabling care. Every day brings new challenges and rewards, making this a truly fulfilling role. You\'ll be key in helping us achieve our goal of being the best in the field.
Shift rotation: 7 day rota, includes evening and weekends/bank holidays. 6 x 8 hr shifts one week and 4 x 8 hrs the next.
Key Responsibilities
* Individual Outcome and Wellbeing: Implement person-centered support plans, promote independence, manage risks, encourage relationships, and ensure legal and best practice compliance.
* Activity Planning: Develop flexible, engaging activity plans that promote autonomy and personal growth.
* Health and Lifestyle: Support individuals in maintaining healthy lifestyles and managing their health needs.
* Communication: Facilitate effective communication and maintain detailed, person-centered records.
* Medication Management: Oversee medication processes and support individuals in self-administration.
* Safety and Environment: Maintain a safe, clean-living environment and adhere to health and safety protocols.
