Job responsibilities Deliver a positive patient experience Oversee and facilitate safe, efficient discharges Collaborate with multidisciplinary teams including doctors, pharmacists, nursing and administration teams Check eDANs and support patients with medication queries and liaising with the pharmacy department as required. Provide patient education and post discharge care Work under time sensitive pressures ensuring discharge cut off times are met Communicate effectively with transport teams to avoid delays Provide reassurance and updates to patients and families Respond swiftly to emerging challenges and make critical decisions in high pressure situations Promote awareness of the Discharge Lounges role in improving patient flow and patient experience. Manage and coordinate the daily operations of the Discharge Lounge in the absence of the senior sister. Clinical Act in accordance to the Nursing and Midwifery Councils Code: Professional Standards Practice and Behaviour (March, 2015) Fulfil the requirements as directed by the Nursing and Midwifery Council to maintain and revalidate registration Assess, plan, implement and evaluate nursing care for patients waiting in the Discharge Lounge Monitor patients conditions while waiting in the Discharge Lounge and responding to any changes Ensure all discharge documentation is complete and accurate Quality Improvement As part of our commitment to delivering safe efficient patient centred care you will contribute to quality improvement initiatives within the Discharge Lounge you will support the development of the pathways working collaboratively with the multidisciplinary teams to streamline processes and reduce delays.
