Our Clinical Pharmacists and Pharmacy Technicians are highly valued and well-integrated members of the practice teams, as well as being supported by the wider PCN team. It is due to the success of our existing team that our practices are keen for us to expand. This advert is for full-time PCN Clinical Pharmacist within the MMWF PCN.

Main duties of the job
Job responsibilities -: Primary Duties and Areas of Responsibility. Undertake SMRs in patients with single or multiple medical problems where medicine optimisation is required. Using tools available to identify and prioritise patients according to clinical risk and contract requirements.
Review the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines ( medicines optimisation). Make appropriate recommendations to clinicians or action recommendation for medicine improvement as appropriate.
Work with care home staff to improve safety of medicines.
Provide patient facing and telephone clinics for those with questions, queries, and concerns about their medicines in the practice.
Review the use of medicines most associated with unplanned hospital admissions and re-admission through audit and individual patient reviews. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups.
Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches. This might include risks that are patient related, medicine related, or both.
To support public health campaigns & work align with QOF/IIF Schemes. To provide specialist knowledge on all public health programmes available to the public.

Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines.
Work with care home staff to improve safety of medicines.
Provide patient facing and telephone clinics for those with questions, queries, and concerns about their medicines in the practice.
Answers medicine related enquiries from GPs, other practice staff, other healthcare teams ( community pharmacy) and patients with queries about medicines. Suggesting and recommending solutions. Providing follow up for patients to monitor the effect of any changes.
Review the use of medicines most associated with unplanned hospital admissions and readmissions through audit and individual patient reviews. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups.

Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components ( advice on treatment pathways and patient information leaflets).
Analyse, interpret and present medicines data to highlight issues and risks to support decision making.
Undertake simple audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the practice team.
Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.
Provide education and training to primary healthcare team on therapeutics and medicines optimisation.
Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.

Person Specification
Qualifications
Essential
4. Masters/Degree in pharmacy (MPharm)
5. Completion of CPPE PCPEP (or equivalent) pathway, or willingness to undertake this.
6. Professional Registration
7. Mandatory registration with General Pharmaceutical Council
Desirable
8. Membership of the Royal Pharmaceutical Society
9. A member of or working towards Faculty membership of the Royal Pharmaceutical Society
Experience
Essential
10. An appreciation of the nature of GPs and General Practice
11. An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
12. Excellent interpersonal, influencing and negotiating skills
13. Excellent written and verbal communication skills
14. Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences ( patients)
15. Good IT skills
16. Able to obtain and analyse complex technical information
17. Recognises priorities when problem solving and identifies deviations from the normal pattern and can refer to seniors or GPs when appropriate
18. Able to work under pressure and to meet deadlines
19. Produce timely and informative reports
20. Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers
