Your role will encompass c ompleting complex asse ssment and treatment plans, developing and leading group education and treatment, and Occupational & physiotherapy service development. The post will primarily involve working with elderly patients and their families with a focus on goal-orientated discharge planning within their homes .

Detailed job description and main responsibilities
In this rewarding role, you'll support adults in their own homes, care homes and remotely, managing complex cases and creating meaningful, person-centred intervention plans to reduce the impact of disability and ill health.
The successful applicant will be s u pported and upskilled to gain experience in a leadership role to include the following responsibilities;
Complex clinical assessments - Moving and Handling and seating specific assessments. Equipment provision and comprehensive Moving and Handling risk assessments and handovers.
S upervision of junior staff Rehab assistants, PT’s and OT’s.
Individual caseload management and supervision of team caseload management.
C ompleting return to work and managing sickness as per policy.
Carrying out detailed and timely appraisals in keeping with KPI’s
Lead on therapy input in the weekly MDT and caseload workshops.
T he post holder will be expected to streamline processes to su pport efficiency within the Integrated Therapy Service.
Service development; a review of current occupational therapy services and an innovative approach to develop ing new aspects of the service.
Lead on competency training for Rehab assistants and therapy staff.
Completing Audits
Supporting patient allocations with the admin team and reviewing new patients in the service.
Person specification
Qualifications
* Diploma or Degree in Occupational Therapy.
Professional Registration
* HCPC registration
* Member of the RCOT.
Skills
* Good awareness of a range of clinical conditions and their management
* Research, teaching and leadership skills.
* Use of own car for work
* Knowledge of additional specialist areas through in-house training & short courses.
Experience
* Experience in previous rotational post or community-based position
* Moving and Handling experience- risk assessment and moving and handling documentation
* Experience of leading a team

* Ability to use Microsoft word, excel and power point.
