Job summary
We have an exciting opportunity for an Extended Clinical Practitioner to join Shakespeare Road Medical Practice, a forward-thinking and innovative GP Practice providing primary care services for over 13,000 patients in Basingstoke.
We are looking for an enthusiastic, caring, and motivated professional with excellent clinical, organisational, and communication skills, who can work autonomously.
The post holder will be an experienced Nurse or Allied Health Professional who, acting within their professional boundaries, will support our Home Visiting Service and Same Day Access Service. The post holder will also hold their own clinics and demonstrate critical thinking in clinical decision-making. They will collaborate with the general practice team and extended primary care team to meet patient needs and support policy and procedures. They will seek to develop innovative concepts, models, methods, and practices to enhance primary care services.
Main duties of the job
To provide high-standard, complex, and enhanced care for patients, utilizing advanced clinical judgement, skills, and knowledge. Extended Clinical Practitioners will consult with patients, families, carers, and the multi-professional team to assess needs and develop and evaluate complex care plans within their expertise. They will critically analyze clinical problems, apply relevant evidence, and use advanced assessments, diagnostics, interventions, and equipment to make informed decisions.
They will deliver complex clinical care in dynamic environments, adapting to change, different care models, innovation, and evolving technologies. Recognizing their practice boundaries, they will know when and to whom to refer patients, and may delegate tasks to team members, taking accountability for delegated activities.
