An exciting opportunity has arisen for a fixed term contract or secondment as a Cancer Pathway Navigator within the Colorectal Team, to work with the Colorectal Triage and Colorectal Nurse Specialist.
The post holder will act as patient advocate, providing support and information throughout the entire cancer pathway to reduce patient anxiety. You will provide reassurance and information on appointments, interventions/tests, ensuring patients arrive for interventions fully prepared. The post holder will work closely with the MDT coordinator to support and coordinate the care of cancer patients through their complex care pathways, ensuring all patients move through the pathway without unnecessary delays in order to achieve a definitive diagnosis by day 28. The post holder will also track patients on cancer surveillance as part of the introduction of Patient Stratified Follow Up (PSFU).
You will work autonomously and have strong communication skills as well as commitment and compassion for patients and their quality of care and experience. You will be competent in data collation and input, with excellent IT skills across a broad range of systems.

Person Specification
Education
* 5 GCSE's including Maths & English at grade C or equivalent
* Level 3 Health or Social Care qualification (e.g. NVQ or similar)
* ECDL or equivalent experience
* Evidence of continued role development
* Basic counselling
Knowledge and Experience
* Relevant Health or Social Care experience
* Computer literate, with full knowledge of Microsoft packages
* Understanding of person‑centred care
* Awareness of improving outcomes; a strategy for cancer and other national cancer policies
* Experience of co‑ordinating a patient workload
Personal Qualities
* Ability to work flexibly dependant on the needs of the service
* Empathic and caring nature, able to be sensitive to the needs of patients
* Ability to retrieve information from a wide range of sources and in different formats
