Main duties of the job

To provide rehabilitation support services.

To undertake specific skilled support work to aid the rehabilitation of patients in the Community. Treatment may occur in the department, patients’ home or other appropriate setting. Treatment may be provided for Physiotherapy, Occupational Therapy or Speech and Language Therapy.

To carry own patient caseload, assessing and treating routine patients within guidelines agreed by the treating therapist. Monitor progress and make minor alterations to patient treatment programmes as required.

Work unsupervised reporting back patient progress and informing the treating therapist of any problems. Receiving guidance and advice on an almost daily basis.

Detailed job description and main responsibilities

Duties and responsibilities:

1.To gather and collate relevant patient information to support assessment and treatment.

·Obtain biographical and clinical information from sources such as electronic patient records.

·Take a simple case history.

·Explore external sources of information related to the patient.

2.To contribute to the assessment process and provide feedback to the therapist.

3.To implement therapy programmes safely and effectively under the guidance of the therapist.

·Deliver individual and group therapy programmes and provide feedback to the therapist.

·Carry out treatment programmes using delegated exercises, techniques, and strategies.

·Monitor patient progress, make appropriate adjustments within agreed limits, and keep the therapist informed.

·Assess and manage clinical risk within your own caseload, seeking guidance when needed.

·Carry out only those procedures for which you have received appropriate training and have been deemed competent.

4.To contribute to the evaluation and adaptation of therapy interventions.

·Provide objective analysis of therapy sessions.

·Use outcome measures to assess progress towards specific goals.

·Adapt treatment programmes based on outcome measurements.

5.To plan and prioritise workload effectively to support service delivery.

·Prioritise own workload under managerial supervision.

·Plan day-to-day tasks efficiently.

·Identify factors affecting prioritisation and support the overall prioritisation of the service.

6.To provide communication and wellbeing support to patients, carers, and professionals as part of the rehabilitation process.

·Help patients, carers, and other professionals understand the aims of care and its impact on wellbeing.

·Use effective listening skills to identify concerns, respond to questions and anxieties, and record and report relevant information to the therapist.

·Demonstrate empathy, sensitivity and strong interpersonal skills, particularly when working with individuals who have barriers to understanding.

7.To provide training and support to staff, carers, and patients.

·Deliver training sessions and problem-solving discussions for staff and carers.

·Develop rapport with patients to encourage interest and participation, assisting in the hand-over of specific programmes, techniques and strategies.

8.To participate in multi-disciplinary team (MDT) activities to support patient care.

·Work alongside the therapist within the MDT.

·Attend meetings under the guidance of the therapist and provide agreed contributions.

·Participate independently in MDT discussions, contributing to assessment, goal setting, and intervention plans, and provide feedback to the therapist.

9.To support audit and clinical governance activities.

·Participate in audit and clinical governance processes.

·Identify potential clinical governance risks, inform the therapist, and gather relevant information as directed.

10.To contribute to service development projects.

·Support project work and contribute to planning and implementation.

·Devise project plans in liaison with the lead therapist and provide evidence-based input aligned with the service’s clinical audit plan.

11.To maintain up-to-date patient records and complete all patient documentation in line with department standards

Skills

Essential criteria

1. Experience of working with the public in a variety of settings and able to demonstrate good customer care. Experience of working in a health care setting, with skills and experience in rehabilitation of patients, general mobility skills and communication Good Verbal and written communication skills

Desirable criteria

2. Good organisational skills Ability to work on own initiative & as part of a team

education

Essential criteria

3. 5 GCSE's at Grade C or equivalent including English language and Maths
