Are you a highly experienced HIU Lead in the East Kent area who would love to work on an innovative NHS programme (High Intensity Use (HIU) Programme)?
The aim of the programme is to change perceptions of people who access healthcare more than expected. The team helps to prevent people from reaching crisis point, empowering them to better self-manage and connect with their surroundings.
What does the role of an Outreach Caseworker involve?
* Working in the community to help identify and address unmet social needs that may be exacerbating physical and/or mental health. Identifying causes of high emergency care service usage.
* Supporting people with complex presentations including alcohol and substance misuse, mental health issues, multiple long-term conditions, and adverse traumatic experiences who may be suspicious of formal support. Your experience and leadership will guide them to access appropriate help.
* Helping to lower stigma within the community and change mindsets and behaviors by building strong relationships with partners and stakeholders, such as A&E departments, Community Mental Health Teams, GP surgeries, housing teams, etc.
What will you bring to the role of Outreach Caseworker?
* Experience in delivering casework within areas of vulnerability such as drugs and alcohol misuse, mental health, and physical health conditions.
* Experience supporting people with complex needs. You should be authentic, relatable, and caring, with the ability to anticipate, identify, and resolve risks and issues.
* Experience in person-centered support planning and dynamic risk assessment.
* Strong negotiation skills and the ability to think on your feet.
* Meticulous record-keeping skills.
* Existing and well-established links to multi-disciplinary teams, community assets, and statutory services in the East Kent area.
