Job Description:
An exciting job role has recently become available at Reigate. They are looking for a new member for Assessments.
Duties:
1. Needs Assessments: Evaluate the care and support needs of individuals to determine their eligibility for services under the Care Act 2014.
2. Risk Assessments: Identify any risks to the individual’s wellbeing and safety, and develop plans to mitigate these risks.
3. Care and Support Planning: Create detailed care plans that outline the support and services required to meet the individual’s needs.
4. Reviewing Plans: Regularly review and update care plans to ensure they remain relevant and effective.
5. Guidance: Offer information and advice to individuals and their families about available services and support options.
6. Counseling: Provide emotional support and counseling to help individuals cope with their circumstances.
7. Crisis Intervention: Step in during crises to provide immediate support and ensure the safety of vulnerable individuals.
8. Safeguarding: Work to protect individuals from abuse, neglect, and exploitation.
9. Multi-Agency Work: Coordinate with other professionals and agencies, such as healthcare providers, to ensure a holistic approach to care.
10. Referrals: Make referrals to other services and agencies as needed to provide comprehensive support.
11. Documentation: Keep detailed records of assessments, care plans, and any interventions or support provided.
12. Reporting: Write reports and maintain accurate documentation to support decision-making and ensure accountability.
What we are looking for in a Social Worker:
1. Social Work England registration.
2. Eligible to work in the UK.
3. Hold a full UK licence.
4. Post-qualified experience.
5. Degree level or equivalent in Social Work.
6. Twice weekly payroll.
