As a Health Care Support Worker, you will support the delivery of nursing care to clients within a clinical setting and/or home environment, as part of a team and under the direction of a Registered Nurse.

Main duties of the job This is an ideal opportunity for a Health Care Support Worker who is seeking to broaden and utilize their range of skills and knowledge to nurse people in their own environment as part of a multi-disciplinary team. The post holder will participate in the delivery of skilled, autonomous nursing care to patients within their own home.
The Llandrindod District Nursing Team are an integral part of the Virtual Ward that aims to prevent unnecessary admissions to hospital and facilitate early discharge home.
This postinvolves lone working and the ability to travel independently is essential.

Person Specification Experience Meets all essential criteria
Working within a multidisciplinary and multi-agency setting
Meets all desirable criteria
Qualifications and/or Knowledge NVQ3 in Health or Social Care or equivalent demonstrable experience in a Health or Social Care setting
ECDL or equivalent level of computer skills
To be aware of the needs of the patients and their relatives/carers and have knowledge of voluntary and statutory services
Meets all essential criteria
Can demonstrate good interpersonal and communication skills both written and verbal
Ability to work within a multidisciplinary team
Ability to manage stressful situations in self
Ability to travel between sites in a timely manner
Can explain the importance of care monitoring, audit and evaluation of fundamentals of care

Ability to work alone under the supervision of Clinical Lead
