The Role
As Nurse Case Manager, youll take ownership of complex care packages for adult clients, ensuring safe, high-quality delivery while developing and supporting care teams.
Key Responsibilities
* Managing and reviewing complex care packages in the community
* Leading clinical quality, governance, and best practice
* Delivering specialist training (e.g. tracheostomy, ventilation)
* Assessing and signing off staff competencies
* Coaching and mentoring care teams
* Building strong relationships with clients, families, and MDTs
About You
* Registered Nurse with NMC PIN
* Experience in complex care (e.g. tracheostomy, ventilation, spinal injury)
* Passion for developing others and improving care standards
* Strong communication and organisational skills
* Confident working independently in a community setting
* Full UK driving licence and access to a vehicle
