As a Substance Care Practitioner in the Substance Care Team (SCT), you will be involved in alcohol and drug screening, brief interventions, fibro-scanning patients, and treating acute alcohol withdrawal following NICE-based protocols for alcohol misuse. You will also deliver Substance Care training for hospital staff.
As an SCT member, your responsibilities include:
1. Improving identification and management of substance misuse across hospital wards to reduce A&E attendances and inpatient admissions.
2. Providing assessment, specialist advice, and treatment for patients with substance misuse issues.
3. Supporting and training staff to enhance their skills and confidence in managing substance misuse.
4. Facilitating safe transitions of care between hospital and community settings, ensuring seamless pharmacological care transfer.
5. Promoting recovery opportunities through effective care planning.
6. Liaising with other health and social care professionals to ensure coordinated, high-quality care.
Key requirements
Clinical:
1. Utilize SCT screening in assessments in emergency and ward settings.
2. Follow referral procedures for timely clinic referrals.
3. Deliver high-quality assessments and evidence-based interventions.
4. Conduct fibro-scanning and liaise with the medical team about outcomes.
5. Implement NICE-based treatment programs for alcohol management in acute settings.
6. Ensure safe transition of care for detoxification and aftercare, maintaining abstinence.
7. Enhance care pathways between hospital and community.
8. Provide advice with MDTs.
9. Be a visible point of contact for patients, visitors, and staff, advocating patient needs.
10. Maintain and update patient records and care plans.
11. Organize MDT discussions on patient management.
12. Participate in audits and quality improvement projects.
13. Signpost patients to external agencies.
14. Review and help embed assessment, treatment, and care policies.
15. Provide specialist support to the clinical team for complex cases.
