Job Summary
The Care Coordinator will oversee day-to-day care coordination, manage client cases, ensure effective communication between caregivers and clients and provide exceptional customer service.
Duties
Care Scheduling
* Coordinate and schedule care visits for clients.
* Ensure all appointments are booked in a timely and efficient manner, based on client needs and staff availability.
Staff Coordination
* Maintain effective communication with care staff to confirm their availability.
* Ensure accurate shift allocation and provide updates regarding any changes to client care needs.
Client Liaison
* Communicate with clients and their families to understand care requirements.
* Resolve any concerns and ensure client expectations are consistently met.
Record Management
* Maintain accurate and up-to-date records of care schedules, client preferences, and other relevant data.
* Ensure compliance with regulatory and organizational standards.
* Collaborate with healthcare providers to develop and implement individualised care plans.
* Monitor and follow up on care plans to ensure adherence to treatment protocols.
* Assist in administrative tasks such as scheduling, data entry, and maintaining office supplies.
Qualifications
* Experience in care coordination and case management
* Strong communication skills
* Familiar with home care procedure.
* Ability to work independently and collaboratively.
