This is a key leadership role responsible for the clinical oversight, operational coordination and ongoing development of the UCR service. You will support the delivery of high-quality, responsive care to patients in their own homes, helping to prevent unnecessary hospital admissions and facilitating timely interventions for those with urgent healthcare needs. This is a full-time position working 37.5 hours per week, ideally 08:00 to 16:00 Monday to Friday, with occasional weekend working as required. Main Responsibilities Provide clinical leadership and operational oversight for the UCR team. Manage the day-to-day delivery of the service, ensuring safe, effective and patient-centred care. Undertake advanced assessment, planning, implementation and evaluation of care for patients with complex needs. Act as an autonomous practitioner, making sound clinical decisions and delegating appropriately. Support, supervise and develop team members, promoting high standards of practice. Ensure all documentation is completed in line with NMC standards and HCRG Care Group policies. Monitor service performance and contribute to continuous improvement initiatives. Respond to urgent issues involving both patients and staff, providing effective leadership during challenging situations. A full list of responsibilities can be found in the attached job description. The Ideal Candidate The ideal candidate will have: Current registration with the Nursing and Midwifery Council (NMC) Evidence of continuing professional development and education at degree level or equivalent Leadership or management qualification, or willingness to work towards one Teaching and assessing qualification or equivalent experience Significant post-registration experience, including community nursing experience Experience of leading teams and managing services Experience of caring for patients with complex and long-term conditions Strong clinical decision-making and organisational skills A full UK driving licence and access to a vehicle for work purposes
