Overview
You will work alongside and in partnership with individuals, families and carers, using a range of interpersonal and communication skills to assess complex psychosocial need, set objectives, develop care plans and deliver appropriate interventions, facilitating safe hospital discharges. You will complete Urgent Needs Assessments, Mental Capacity Assessments and collaborate with external services to access resources needed to meet identified care and support needs. You will benefit from a balance of autonomy and support, responsible for the day‑to‑day management of your caseload, with regular supervision and full support from your Team Manager.
Key Details
The role is an 8‑day (37 hours) week on a 12‑month fixed‑term contract basis, based predominantly at Wansbeck General Hospital, with flexibility to cover other hospitals across Northumberland.
Qualifications
* Degree in Social Work & registered with Social Work England
* Successfully completed the UK Skills for Care ASYE programme
* Valid UK driving licence and ability to use a car for work purposes
