Aneurin Bevan University Health Board support flexible working.
Please be advised that there is a temporary top up for Bands 2 and 3 to reflect the incorporation of the top up to the living wage of £12.60 per hour - £24,638 per annum.
This temporary top up will be in place until the annual pay uplift for 2025/26 is confirmed.

Job Overview
This is an exciting opportunity for a motivated behaviour change practitioner to work as part of a growing, dynamic and busy team contributing to improving the health and well-being of the population of Gwent. This will include providing specialist behaviour support and advice to people who want to quit smoking, as well as being equipped with the ability to support people to make changes to their diet, physical activity and alcohol use where appropriate.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job
The post holder will work independently in a variety of community, primary care and secondary care settings across Gwent, providing support and interventions to individuals and groups. They will be a core part of a place-based neighbourhood team and will support building the capability of behaviour change amongst professionals and across neighbourhood teams through the delivery of formal and informal training. They will apply relevant theory and behavioural science approaches to inform and deliver a range of health promoting interventions, including immunisation and screening programmes, and advise on approaches to service delivery, communications and engagement.
The post will be driven by the needs of the population and the business and will require the post holder to work evenings and weekends.

Qualifications And Knowledge
Essential criteria
* Degree or recognised professional qualification in health or social care or/and equivalent level of previous experience
* Understanding of the principles and practice of client confidentiality, health related behaviour and change strategies
* Understanding of clinical governance systems and processes and the requirements of safeguarding policies and procedures
* Knowledge of local area
Desirable criteria
* Certificate in Counselling, WNB Counselling Skills
* Health Promotion qualification e.g., Certificate, WNB Diplomas, Open University
* Knowledge of health promotion theories and approaches, particularly behaviour change, Making Every Contact Count (MECC)
* Knowledge of smoking cessation guidelines and practice wider tobacco control policy
* Has used NHS Databases e.g., CWS
* Awareness of policies and procedures relating to dealing with confidential data
* Knowledge of care related procedures and practices
Experience
Essential criteria
* Experience of providing client/patient centred services in a health or social care setting OR Experience of health education / one to one counselling on health issues
* Experience of working as part of a multi-disciplinary team
* Experience of communicating with the public and with varied levels of staff
* Experience of dealing with confidential, sensitive information, appropriate maintenance and storage of records
* Experience of using Microsoft Office
Desirable criteria
* Experience in delivering specialist behaviour change support to clients in a group setting
* Experience of delivering health related presentations and training to clinical staff
* Significant and relevant work/social experience, which has utilised skills and knowledge that could be of benefit to the post
* Experience of working in the NHS, social care or community setting, maternity service
