What You’ll Do:
1. Deliver high-quality holistic care to prevent unnecessary hospital admissions, alongside specialist nursing interventions
2. Support timely discharges and promote recovery at home
3. Collaborate with a multidisciplinary team including Physiotherapists, OTs, Nurses, and Support Workers
4. Engage patients and families using health coaching techniques to set realistic, empowering goals

Main duties of the job
The post holder will co-ordinate activities in the CRT team ensuring effective communication is always maintained, providing support to registered and unregistered staff within the ward/department.
This post will play a key role in the assessment of patient care needs and the development, implementation, and evaluation of programmes of care.
The post holder will act as a role model, promoting quality evidence-based practice, developing the nursing team, and ensuring effective management of the ward/department in the absence of the Team Leader.
The post holder will work in partnership with the Team Leader and the team to ensure that high standards of nursing care are always provided.
The post holder will provide day to day support to the non-registered clinical support team, consisting of Nurse Associates and Generic Support Workers within the York Community Response Team.

Education, Qualifications and Training
Essential criteria
9. Registered Nurse
10. Recognised teaching / assessing qualification
11. Evidence of continuing professional development and implementation in clinical practice in the community within the last 12 months.
Experience and Knowledge Required
Essential criteria
12. At least 3 years experience at Band 5 or equivalent
13. Insight into clinical speciality - community
14. Insight into rehabilitation and re-ablement issues
15. Insight into current issues in nursing / professional issues
Skills and Attributes
Essential criteria
16. Excellent organisational and prioritising skills
17. Ability to communicate effectively with patients and the multi-disciplinary team, dealing with often difficult, complex situations, responding in a calm and timely manner
18. Reliable, adaptable to change and quick to learn and have the ability to work on own initiative or as part of a team
19. Good verbal and written communication skills including excellent telephone manner
20. Ability to teach, supervise and assess registered / un-registered staff
