Patient Pathway Coordinator Lead, Specialised Cardiology

The second part of the role focuses on the outreach programme ELOPE which leads and supports management of CVD risk factors and health promotion in the community for both primary and secondary prevention. This involves working with schools, employers and media

This role will be split into two main areas of focus - one supporting existing clinical services for cardiovascular disease (CVD) prevention, and one supporting the CVD prevention outreach programme of East London Cardiovascular Disease Prevention (ELOPE).

The existing clinical prevention services include CVD risk & lipids, clinical pharmacology and hypertension, and cardio rehab. as well as closely with the Integrated Care Board. There is also an expectation to support with funding applications, for research grants or other bids


Main duties of the job

You will be working with consultants, nurses and community staff such as social prescribes to provide high quality care to patients. East London Cardiovascular Disease Prevention within the Specialised Cardiology directorate, alongside seven other specialist services.

Person Specification


Qualifications

* Educated to degree level
* Accredited Project Management Qualifications or equivalent


Experience

* Experience of working at an executive assistant level within an appropriate environment.
* Experience using Microsoft office packages to produce report, spreadsheets and presentations
* Experience in service improvement
* Experience with, and a creative approach to, problem solving and responding flexibly to planning and change.
* Experience or demonstrated involvement with health promotion activities
* Experience in project management
* Experience in quality improvement


Knowledge and Skills

* High level of numeracy and ability to research, benchmark, analyse and present relevant financial and other data, and strategic and procedural information
* Excellent written and oral communication skills, including internal and external report writing.
* Excellent IT skills including effective use of spreadsheet, database and similar software to analyse and present complex information.
* High-level interpersonal skills including motivational negotiating, influencing and relationship building and ability to work creatively with diverse teams.
* Knowledge of outpatient transformation within the NHS landscape
* Knowledge of current challenges facing the NHS


Other

* Initiative and drive to achieve results
* Attention to detail and conscientious.
* Flexible approach to working and a desire to develop knowledge.
* Ability to foster effective working relationships with colleagues and contacts at all levels, both internal and external.
