Key Responsibilities * Assist patients/residents with personal care, including washing, dressing, toileting, and grooming. * Support with mobility, repositioning, and use of aids and equipment. * Assist with serving meals and drinks, encouraging adequate nutrition and hydration. * Observe and record patients’ physical and emotional conditions, reporting any changes to the nurse or supervisor. * Help maintain a clean, safe, and comfortable environment for patients. * Take and record vital signs (temperature, pulse, respiration, blood pressure) as required. * Support with rehabilitation and daily activities to promote independence. * Communicate effectively with patients, families, and the multidisciplinary team. * Maintain confidentiality and follow infection prevention and control policies. * Complete care documentation accurately and promptly.
