About Us
Shotfield Medical Practice is composed of four GP partners, five salaried GPs, a Nursing team including HCA support and a comprehensive admin support team providing primary care services to a registered population of approximately 13000 patients across the Wallington, Carshalton and Beddington areas.

Job Responsibilities
Clinical Duties
* Work as an autonomous practitioner within the multi-disciplinary team to provide Practice Nursing services including cervical cytology, chronic disease management (asthma, diabetes, COPD), wound management, ear irrigation, vaccinations, travel health advice, and other general treatment room services.
* Provide assessment, implementation, and evaluation of clinical care packages.
* Work with patients and healthcare professionals to deliver packages of care.
* Advise on management of chronic diseases and appropriate therapies.
* Refer patients to other disciplines as needed.
* Undertake clinical audits and assist with QOF targets.
Administrative Duties
* Maintain accurate notes on EMIS Web.
* Monitor and order medical supplies.
* Ensure medical equipment maintenance.
* Stock clinical rooms appropriately.
* Assist with patient recall and follow-up systems.
Communication
* Maintain effective relationships with team members and patients.
* Communicate effectively verbally and non-verbally.
* Assess patient consent and communicate via emails.

Respect patient confidentiality and privacy, disclosing information only to appropriate personnel when necessary for safety.

Person Specification
Qualifications
* Registered Nurse, Adult Nurse Diploma/Degree, Qualification in Asthma and COPD, NMC Registration.
* Desirable: Diploma in Diabetes.
Experience
* Minimum 1 year experience, including cytology, chronic disease management, wound care, immunizations, travel advice, hypertension, safeguarding.
* Desirable: experience in population health, contraception, learning disabilities, mental health checks.
Knowledge & Skills
* Initiative, independence, teamwork, knowledge of NICE guidelines, time management, communication skills, working under pressure.
* Desirable: experience with EMIS Web, GMS/PMS contracts, auditing.
Personal Qualities
* Flexibility, organization, commitment to equality, team player, desirable: driver’s license.
