- Provide support and guidance to patients and their families regarding health conditions and treatment protocols
- Collaborate closely with nurses, physicians, and other healthcare providers to ensure coordinated patient care
- Maintain accurate and up-to-date patient records in compliance with regulatory standards
Qualifications:
- Previous experience in a healthcare setting preferred
- Excellent communication skills in English, both verbal and written
- Familiarity with care home or home care environments is advantageous
Nice-to-Have Skills:
