Job overview

This exciting role is to work as a Clinical Support Worker Higher Level within the multidisciplinary team working across Redcar PCN within Tees Community Nursing.

This is an opportunity for those who are interested in working in the local community and being part of an exciting new model of working in the community localities of Redcar And Cleveland

Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.

Main duties of the job

To work as part of the community nursing team; we deliver a 7 day service, including some weekends.

The post holder will undertake a broad range of care activities, including wound care and venepuncture

The role will require some flexibility and this may require the post holder to work across acute and community in response to service demands.

To support the delivery of patient care and other duties that contributes to the care environment. This might include, for example, having a Link role; supporting therapy and nursing/medical staff.

To support the delivery of a range of duties that support patient care and maintain a suitable working environment.

Knowledge & Skills

Essential criteria

1. Excellent interpersonal, communication & active listening skills. Ability to adopt appropriate styles & methods of communication.
2. Timekeeping, planning & prioritisation skills.
3. Basic IT skills to use electronic patient records & other Trust systems.
4. Analytical and judgement skills to make decisions based on patient / service user interactions and observations and metrics.
5. Working knowledge of legislation underpinning care provision.
6. Ability to demonstrate the importance of holistic person-centred care & related procedures.
7. Understanding of the importance of robust, accurate, clear, timely documentation.
8. Insight into behaviours and skills of a carer and the need for effective teamwork.
9. Awareness of GDPR and the need for confidentiality

Experience

Essential criteria

10. Experience of working in health or social care.
11. Experience of undertaking and recording clinical observations and escalation of changes or findings.

Qualifications

12. Numeracy and Literacy Level 2 or Maths & English (English Language or English literature) GCSE A-C/4-9 or Maths & English O Level/GCE A-C or Maths & English CSE Grade 1 or NARIC/ENIC which compares the qualification to UK GCSE (
13. Care Certificate (if you dont hold this certificate, you would be expected to complete this during your first 6 months of employment)
14. Level 3 vocational qualification or Short courses and completion of competency workbook equivalent to Level 3 in a range of patient and clinical care duties and/or Equivalent knowledge and experience in a range of clinical care duties equivalent to Level 3. -Theoretical knowledge -Experience of a range of care and related procedures -clinical observations -knowledge of relevant legislation
