East Lothian Health and Social Care Partnership is seeking two experienced and motivated Social Workers to join our newly established Prevention and Early Intervention multi-disciplinary team.
This innovative team works collaboratively with NHS colleagues and partners from Community First, hospital flow and discharge services, reablement, social work, and care-at-home teams. Our core focus is to:
1. Prevent unnecessary hospital admissions
2. Support timely and safe discharges
3. Reduce the impact of unscheduled care
4. Promote recovery at home
As a Social Worker in this team, you will:
5. Adopt an “In Reach” approach within Edinburgh hospitals
6. Carry out assessments for individuals in the community
7. Deliver responsive, person-centred support to enable independence and recovery
Working Pattern
Currently Social Work element of this service operates Monday to Friday. There is the Potential for future seven-day working, with weekend shifts anticipated up to two weekends per month.
While eligible for the Council’s flexible working scheme, the nature of this role demands a high level of agility and responsiveness. As such, set working days cannot be guaranteed, and on-site working will be essential, with limited scope for homeworking.

Please Note:
All home/hybrid workers must live in the UK. Where an application for home-working is supported, it will be subject to a satisfactory trial period. Employees will be expected to attend Council offices for the duration of induction, and be on site in East Lothian workplaces, as required. Employees with a hybrid working arrangement in place will be office based and able to work from home, with permission, on an ad hoc basis.
