LSFRC is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission. The role of our Support Worker plays a crucial role in fulfilling our customers' expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Workers are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professionals and maintaining excellent record keeping in line with our compliance management. You will also be required to oversee support planning, Risk Management Plans, and Key-working.
