An exciting opportunity has arisen to join HerefordshireCommunity Learning Disability Teams as a Band 5- 6 Community LearningDisability Nurse based at Liverpool Court, Commercial Road, Hereford.
The successfulapplicant will become part of a small but tenacious and committed team who arepassionate in ensuring that individuals with a learning disability receiveequitable health care alongside managing an effective caseload and maintainingregular visits for service users and their carers.
The team arecurrently involved in some exciting service developments including STOMP,Dementia pathway, transitions pathways to better meet the needs of individualswith learning disabilities. We welcome creativity, enthusiasm, and passion inthis team with a go for it attitude to best meet patients health needs.
You willreceive regular individual and group supervision, on-going CPD & training,and support within this role to help further develop yourskills and competencies in this specialist field.
Weoffer flexible working and full equipment provided for home working. Due to therural landscape of the county, being a car driver is essential but there alsoare good public transport links to the main base.

Main duties of the job
1. Actively manage aneffective caseload, maintaining regular visits for service users.
2. Facilitate/teacheffective systems to carers to enable the transfer of skills required for themanagement of service users healthcare needs.
3. Facilitate HealthAction Plans for each service user on caseload, to enable access to the fullrange of primary and secondary care services available.
4. Promote and maintaingood working relationships and systems of effective communication throughliaison with service users, carers, and other professionals / agencies.
5. Maintain accurate andup to date clinical records in line with Trust/NMC standards of practice.
6. Record activity to meetthe requirements of Patients Information Management System.
7. Attend team andmulti-professional meetings as appropriate.
