Join to apply for the Band 5 Community Nurse role at HCRG Care Group. 12 month FTC, 37.5 hours per week. City Community Team is seeking a Community Nurse to join our supportive and dynamic team at Wilton Health Centre. We provide complex care to vulnerable individuals, often in challenging environments, ensuring they receive high-quality support closer to home.
The role offers the chance to work with a multidisciplinary team and make a real difference in local health and care delivery.
What You’ll Do (Responsibilities)
* Care for patients and their families in line with our high service standards, sharing best practice and continuously improving how we work to better support the local community.
* Utilise strong communication and organisational skills with a flexible approach to deliver positive patient experiences at every stage of the healthcare journey.
* Collaborate with a multidisciplinary team, including Occupational Therapists, Physiotherapists, and Consultant Practitioners, to provide holistic care.
What We’re Looking For (Qualifications / Skills)
* Registered Nurse with NMC/1st level registration.
* Ability to work independently and as part of a multidisciplinary team; strong communication and organisation skills; flexible and resilient under pressure.
* Full UK driving licence with access to a car for work purposes.
* Confident in using IT systems and proficient with Microsoft Office (Outlook, Word, Excel).
Working Pattern
Shift patterns include: 7:00–15:00, 08:30–16:30, and 14:00–22:00.
