About the Role: Reporting to the Clinical Care Manager, the Community Lead is responsible for leading and delivering care plans in the home and ensuring the implementation of residents care. The Community Lead is involved in all aspects of supporting our Clinical Leads and health and safety, prioritising relationship centred care and best practices to ensure our residents are living a happy and fulfilled life. Your Responsibilities: Our Community Lead oversees high-quality care within our home. They will manage and motivate the community team, ensuring effective staffing, training, and performance, while acting as a role model for our values. They will lead the delivery and evaluation of person-centred and clinical care, support admissions and care planning, and ensure safe medication practices. With a strong focus on health, safety, and compliance, they will help maintain a safe environment, support audits, and foster positive relationships with residents, families, and team members.
