We are seeking an experienced and dynamic Advanced Clinical Practitioner(ACP) to join our multidisciplinary Enhanced Primary Care Mental HealthService. The post holder will play a key role in delivering high-quality,patient-centred care by providing comprehensive physical health assessments forindividuals with long-term health conditions and have a mental healthcondition, and conducting annual physical health checks for patients undershared care arrangements.
Main duties of the job
The ACP will work autonomously and as part of a team to support earlyidentification of physical health issues, provide ongoing management ofco-existing physical and mental health conditions, and ensure equitable accessto healthcare for this vulnerable population. The post holder must be aqualified independent prescriber and will utilise their advanced clinicalskills to diagnose, manage, and prescribe as appropriate. The ACP will beclinically responsible to prescribe in the absence of a GP.

Job responsibilities
Clinical Practice:
* Conductphysical health assessments and screenings for patients with long-term mentalhealth conditions, including cardiovascular risk assessments, diabetes checks,respiratory assessments, and metabolic monitoring.
* Undertakeannual physical health checks for patients receiving shared care (e.g.,antipsychotic monitoring in partnership on behalf of primary care).
* Interpretclinical findings and initiate appropriate treatment plans or referrals.
* Prescribemedications safely and appropriately, in line with local and nationalguidelines.
* Manageand review physical health issues in collaboration with mental health andprimary care teams.
* Promotelifestyle advice and health education (e.g., smoking cessation, diet, physicalactivity).
* Maintainaccurate and up-to-date clinical documentation in line with local andnational policies.
Leadership and Service Development:
* Contribute to service planning and quality improvement initiatives focused on integrating physical and mental healthcare.
* Support junior staff and contribute to the clinical supervision and training of colleagues where appropriate.
* Liaise effectively with GPs, community teams, pharmacy, and other healthcare professionals to ensure continuity of care.
Governance and Professional Practice:
* Adhere to clinical governance frameworks including risk management, incident reporting, and audit.
* Maintain own professional development in line with NMC/HCPC registration and revalidation requirements.
* Follow all local and national policies relevant to the ACP role, prescribing, and mental health services.
Person Specification
Experience
* Significant post-registration experience in mental health or physical health settings.
* Experience in working with patients with serious mental illness (SMI)
* Experience of audit, service evaluation, or quality improvement projects
Knowledge and Skills
* Competence in managing long-term conditions and conducting physical health assessments
* Strong communication, clinical decision-making, and risk management skills
* Experience of using EMIS Web Clinical System
* Shared Care Prescribing
Qualifications
* Registered Nurse, Allied Health Professional, or other relevant healthcare professional with valid NMC/HCPC registration
* MSc in Advanced Clinical Practice or equivalent
