What You Will Do:
* Provide frontline nursing care and health support to individuals experiencing homelessness.
* Carry out health assessments, basic treatments, and referrals to appropriate services.
* Deliver street outreach and participate in regular street rounds to engage hard-to-reach individuals.
* Work closely with local health providers and community organisations to coordinate ongoing care.
* Promote health awareness, harm reduction, and preventative care guidance.
* Maintain accurate, timely clinical records and outcome reports.
