<div><p>We are looking for a practice nurse to join our friendly,supportive and committed Practice Team. The successful individual will beresponsible for the delivery of practice nursing services, working as part ofthe practice multidisciplinary team and delivering care within their scope ofpractice to the our patient population. The practice nurse will beresponsible for a number of clinical areas within the treatment room,health promotion and prevention.</p><h3>Main duties of the job</h3><ul><li>Provide a holistic approach to wound management and implement wound care in line with current evidence based guidelines, including Dopplers and removal of sutures</li><li>Undertake Cervical Cytology</li><li>Offer lifestyle advice as appropriate.</li><li>Undertake ECGs</li><li>Undertake Venepuncture</li><li>Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions</li><li>Carry out Routine immunisations/Childhood immunisations/travel vacinnations</li><li>Provide Chaperoning and assisting patients where appropriate who are being examined by another clinician</li><li>Assisting GPs with minor surgery and other clinical procedures</li><li>Requesting pathology tests, for example urine culture, swabs</li><li>Following agreed clinical protocols with referral to senior nurses or GPs as appropriate</li></ul><h3>About us</h3><p>With over 18,000registered patients Combe Coastal Practice is the largest GP Practice in NorthDevon. Our main surgery is in Ilfracombe with branch surgeries at Combe Martinand Woolacombe.</p><p>Combe CoastalPractice is a busy teaching Practice with 12 GP partners and 6 salaried GPs. Wehave strong management, nursing teams and admin teams.</p><h3>Job responsibilities</h3><p><b>Job Summary:</b></p><p><b>Responsible for providing nursing services to the patients of thepractice including treatment, screening and advice and vaccinations in order tomeet the health care needs of the practice population.</b></p><p>Workingin accordance with the NMC Code of Conduct, the post holder will provideassessment, treatment, screening, health education services and advice topatients.</p><p>The postholder is responsible for ensuring the delivery of the highest standardsof safe and effective nursing care to the whole practice population.</p><p>Clinically,the focus of the role is the delivery of evidence-based practice forpatients as required by the Gps, including venepuncture, wound assessmentand dressings, ear syringing, ECGs, cervical screening and Vaccinations.</p><p>Theywill manage the provision of child immunisations to agreed protocols.</p><p>Theywill work collaboratively with the whole practice team to meet the needsof patients, with reference to practice agreed protocols and guidelines,providing nursing treatments to patients both independently and inparticipation with nurse colleagues and GPs.</p><p>Toprovide general and specific health screening, including cervical cytologyand new patient checks, to agreed protocols with referral to Gp ifnecessary.</p><p>Toadvise patients on travel healthcare, to plan and give holidayimmunisations schedules and provide other prophylactic injectionprogrammes as required.</p><p>To undertake health education using all available resources and to organisehealth education where needed</p><p>Toensure adequate Treatment Room supplies are held, and that all equipmentin this area is clean and well maintained.</p><p>Toorganise and maintain appropriate registers and accurate records.</p><p>Tomaintain professional development by attending courses and study days asagreed with GPs/Management team</p><p>To carryout community work as required by the GPs.</p><p>Toparticipate in clinical audit activities.</p><p>Toparticipate in the education and training of other Practice Nurses orother disciplines within the Practice.</p><p>To befully aware of personal responsibilities regarding Health & Safety inthe Practice</p><p><b>General Duties</b></p><p>Maintainthe highest standards of infection control at all times, maintaining ownwork areas in a tidy, clean, safe and free from hazards. Initiation ofremedial/corrective action where needed or escalation to the managementteam.</p><p>Provide comprehensive travel health advice for patients prior to travel includingmalaria prophylaxis, safe sex, sun protection, food hygiene, first aid andemergency medication, health insurance and vaccinations</p><p>Administerchild and adult immunisations and vaccinations in accordance with nationaland local programmes. Be competent in anaphylaxis and resuscitationtechniques. Ensure safe storage, rotation and disposalsof vaccines anddrugs and that adequate stock levels are maintained including the orderingof vaccinations.</p><p>Undertakeprocedures including: venepuncture, BP management, urinalysis, pregnancytesting, ECGs, cervical cytology, wound management, Dopplers, removal ofsutures, ear care and syringing.</p><p>Providesupport and chaperoning to the doctors for minor surgery and otherprocedures such as coil fittings</p><p>Promotehealth and wellbeing, giving general education advice on diet,contraception, smoking, chronic disease management, exercise etc.</p><p>Traveladvice and vaccinations.</p><p>Initiateand carry out programmes of health screening as agreed with the practiceclinical team</p><p>Advise,support and encourage individuals, families and groups to address issueswhich affect their health and social wellbeing</p><p>Demonstrateawareness of safeguarding issues and promoting the protection ofvulnerable adults and children</p><p>Carryout spillage control and decontamination procedures</p><p>Contributeto the practice achieving its PMS contract and QOF obligations</p><p>Safemanagement of sharps procedures including use, storage and disposal</p><p>Ensureall data protection requirements are met when gathering, recording andstoring patient data</p><p>Liaiseand maintain good working relationships with all members of the practiceteam. Work within the multi-disciplinary team within the practice andacross the wider health system</p><p>Workwithin the latest NMC Code of Professional Conduct</p><p><b>Health & Safety:</b></p><p>The post-holder will assist inpromoting and maintaining their own and others health, safety and security asdefined in the practice Health & Safety Policy, to include:</p><p>Using personal security systems within theworkplace according to practice guidelines</p><p>Identifying the risks involved in work activitiesand undertaking such activities in a way that manages those risks</p><p>Making effective use of training to updateknowledge and skills</p><p>Using appropriate infection control procedures,maintaining work areas in a tidy and safe way and free from hazards</p><p><b>Equality and Diversity:</b></p><p>The post-holder will support theequality, diversity and rights of patients, carers and colleagues, to include:</p><p>Acting in a way that recognizes the importanceof peoples rights, interpreting them in a way that is consistent with practiceprocedures and policies, and current legislation</p><p>Respecting the privacy, dignity, needs andbeliefs of patients, carers and colleagues</p><p>Behaving in a manner which is welcoming to andof the individual, is non-judgmental and respects their circumstances, feelingspriorities and rights.</p><p><b>Quality:</b></p><p>The post-holder will strive tomaintain quality within the practice, and will:</p><p>Alert other team members to issues of qualityand risk</p><p>Assess own performance and take accountabilityfor own actions, either directly or under supervision</p><p>Contribute to the effectiveness of the team byreflecting on own and team activities and making suggestions on ways to improveand enhance the teams performance</p><p>Work effectively with individuals in otheragencies to meet patients needs</p><p>Effectively manage own time, workload andresources.</p><p>Participate in clinical audit</p><p><b>Communication and Liaison:</b></p><p>The post-holder should recognize the importance of effectivecommunication within the team and will strive to:</p><p>Communicate effectively with other team members</p><p>Communicate effectively with patients and carers</p><p>Recognize peoples needs for alternative methods ofcommunication and respond accordingly.</p><p>As well as the nursing team, thereis a need to work closely with reception and office staff to ensure the smoothrunning of the practice, reporting any problems encountered to the relevantperson and ensuring everyone is aware of the different roles within the nursingteam.</p><p><b>Working Conditions and Risks</b></p><p>Exposureto body fluids, blood, wounds etc.</p><p>Occasionalexposure to aggressive behaviours</p><p>Occasionalrequirement to undertake home visits</p><p><b>Termsand Conditions</b></p><p>Pension Arrangements: You will be auto enrolled into the NHSPensions Scheme subject to terms and conditions of the scheme.</p><p>Health Assessment: This post is subject to asatisfactory pre-employment health assessment</p><p>Criminal Records: The successful applicantwill be required to undertake an Enhanced Disclosure check (Paid for by thePractice). Due to the nature of the work, this post is exempt from the provisions of the Rehabilitation of offenders Act 1974.
