Position Summary
We are delighted to offer an exciting career progression pathway for a motivated and forward-thinking Pharmacy Technician to join our innovative community mental health transformation programme, bridging specialist mental health services and Primary Care Networks (PCNs). Your work will focus on maximising safe and effective prescribing, enhancing pharmaceutical care, minimising avoidable admissions, and improving physical health monitoring for those on the Serious Mental Illness (SMI) register.
Eligibility
We welcome applications from newly qualified Pharmacy Technicians, as well as those who are currently registered or due to register with the General Pharmaceutical Council within the next three months.

Key Responsibilities
* Undertake medicines reconciliation for patients discharged from LPFT services and those transferring from other healthcare providers.
* Ensure the service provided is patient‑centred, safe and supports the cost‑effective use of medicines and resources, and participate in developing innovative ways of working to enhance patient care.
* Provide an inpatient ward and community team clinical medicines management service, which includes efficient safe medicines supply and storage.
* Develop and participate in basic training and teaching activities relating to medicines management across the Trust to other healthcare professionals, service users and carers where required.
* Ensure mental health medication prescribing and monitoring align with local and national guidelines (e.g. HDAT, valproate, lithium, clozapine, depots).
* Manage prescription queries, providing advice on mental health drug use and storage, and respond to medication queries within competency.
