An exciting opportunity has arisen for a dynamic and innovative individual to work as a Patient Navigator within the Colorectal cancer nursing service, within the Surgical Directorate. We are seeking a highly motivated individual who will embrace this exciting and rewarding role.

The post holder will be an integral part of the Colorectal cancer nursing team and will assist, support and co-ordinate the pathway for their patients.

As a patient navigator in the Colorectal cancer service, you will be involved with patients on a suspected or confirmed colorectal pathway, supporting them throughout their cancer pathway by maintaining high patient experience and outcomes. The post holder will help patients navigate their healthcare journey, acting as a single point of contact to provide information, coordinate appointments, and offer support throughout the process. Key duties include signposting patients to the correct services, managing administrative tasks like booking appointments, and helping to reduce the risk of missed appointments and fragmented care. The role supports both patients and clinical teams by ensuring patients are on the right pathway and that necessary information is shared efficiently.

The successful applicant will ensure the efficient channelling and close coordination of the patient pathway ensuring robust communication between patients, relatives, nurses, Surgeons and oncology teams. The post holder will signpost patients and carers to the relevant source of support and information appropriate to their needs.

The post holder will demonstrate excellent organisational skills, be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity. The post holder will require strong organisational and administrative skills.

A requirement of this post is to become a member of the Protection of Vulnerable Groups (PVG) Scheme Prior to appointment.

As from 1/4/2026, the Agenda for Change full-time working week will be reducing from 37 to 36 hours per week. Part time hours will be reduced pro-rata. Please note that the pay scale in this advert will not be changing as a result
