Highly Specialist Physiotherapist - Palliative care

Job Summary
We are seeking to appoint an experienced senior specialist palliative care physiotherapist to work within the Community Specialist Palliative Care service of the Cheshire Wirral partnership. The service includes specialist occupational therapy, physiotherapy, therapy assistant and clinical nurse specialists and is based across the neighbourhood care group within the Community care teams. The post holder will be based at Custom House with Ellesmere Port West CCT.
Responsibilities
The post holder will lead the provision of specialist palliative therapy and end‑of‑life rehabilitation to the people of West Cheshire with malignant or non‑malignant conditions who have a limited life expectancy. They will work autonomously and as part of a team, delivering therapeutic interventions in the patient’s own home and/or when they attend the Hospice as an inpatient or the Living Well Centre. They will provide advice, education and support to patients, carers and other health care professionals, promote advanced care planning, manage complex end‑of‑life symptoms and patient and carer expectations while meeting the integrated care needs of the patient.
Over recent years there has been a lot of development within the service and the successful candidate will help share the team’s drive to take the service forward. The role requires HCPC registered physiotherapists with relevant post‑graduation experience who have a genuine desire to make a difference and relish the opportunity to care for people in their preferred place of care. The holder may lead and support a team of therapists who strive to deliver high‑quality care. As the role involves assessment and treatment of patients within their own home, access to transport for work purposes is essential.

Person Specification
* Essential Qualifications
o Diploma/Degree in Physiotherapy
o Attained or working towards Level 7 Masters study
o HCPC registration
o Member of the CSP
* Desirable Qualifications
o Evidence of a range of specialised and validated postgraduate courses
* Essential Knowledge and Experience
o Specialist scientific and clinical knowledge of rehabilitation and community therapy practice
o Broad experience across various areas of physiotherapy practice including orthopaedics, general medicine, respiratory, neurology and MSK
* Essential Experience
o At least 5 years post‑graduate experience, including previous experience in a variety of rotational areas
o At least 2 years’ experience working within the community setting
* Desirable Experience
o Previous leadership experience
