Assistant Practitioner – Dietetics

Join the Herefordshire and Worcestershire Health and Care NHS Trust to support patient nutrition across community hospitals and home care settings. The role works alongside a registered Dietitian, providing patient assessment, dietetic care plan delivery, and interdisciplinary collaboration.
Main Duties
The main duties of this role include:
* Assist registered Dietitians with nutritional assessments of patients
* Support the delivery of dietetic care plans, monitoring weight, food intake and hydration status
* Provide practical advice on nutrition and food choices under supervision
* Undertake routine reviews of patients
* Act as first point of contact for dietetic queries, escalating complex issues to registered Dietitians
* Support delivery of nutrition training to Trust staff
* Liaise with nursing staff, medical teams and community services for continuity of care
* Schedule patient appointments, home visits and follow‑ups in coordination with the wider team
* Support audit and data collection activities for service outcome monitoring and quality improvement
* Provide general administrative support to the dietetic team
Qualifications
* Foundation Degree in Health and Social Care (or equivalent Level 5 award such as Higher National Diploma) or Higher Apprenticeship in Assistant Practitioner
* GCSE Maths and English (Grade A-C) or equivalent
* Provide original certificates to verify qualifications at offer
Experience
* Significant experience working within a relevant clinical service as a senior Healthcare Assistant/Clinical Support Worker
Additional Criteria
The Trust does not routinely provide sponsorship for work. Employment on a time‑limited visa does not guarantee sponsorship at point of expiry.
