Memory Nurse

An exciting opportunity for a Band 6 Memory Nurse to join our Countywide EMDASS Team in West Hertfordshire. The role requires excellent organisational and communication skills, solid clinical experience and the ability to work independently and within a multi‑disciplinary team.
Main Duties of the Job
• Perform memory assessments for service users suspected of dementia.
• Deliver diagnosis in collaboration with a multi‑disciplinary team.
• Provide treatment and post‑diagnostic support through titration reviews.
• Work between the One‑Stop Pathway and Primary Care.
Job Responsibilities
• Carry out memory assessments using appropriate cognitive tools.
• Develop care plans that promote independent living in the community.
• Complete risk assessments and implement risk management plans.
• Maintain high standards of record keeping and documentation.
• Support transformation of the EMDASS service to improve efficiency.
• Conduct supervision of junior staff and trainees as delegated by the MATS Team Leader.
• Participate in audits and quality improvement initiatives.
• Ensure adherence to patient confidentiality and Trust policies.
Person Specification
Qualifications
* Degree, diploma or equivalent in Nursing (Mental Health Nursing).
* Registration with Nursing and Midwifery Council.
* Two years post‑registration experience, with at least one year in older people’s mental health care.
* Post‑registration course in Dementia.
* Commitment to becoming a non‑medical prescriber.
* Understanding of end‑of‑life care in dementia.
* Pre‑ or post‑registration training/short courses in mental health and/or older people’s care.
* Community and In‑Patient nursing experience.
Experience
* Knowledge of legislation related to mental health and social care (e.g. Mental Health Act, Mental Capacity Act, National Service Framework, National Dementia Strategy, Care Programme Approach).
* Good knowledge of Microsoft Word, PowerPoint and Excel.
* Ability to manage distressing and challenging situations.
* Expert clinical skills in dementia care.
* Experience in research.
* Experience in audit.
