Working as part of the PCN's multidisciplinary team, the post holder will support GP practice patients in managing their health conditions around what matters to them. Using coaching skills, patients will be supported to address issues and promote lifestyle medicine to develop skills, knowledge, and confidence to improve health outcomes, wellbeing, and quality of life.
Main duties of the job
The postholder will use coaching techniques to provide one-to-one and group guidance, advice, and coaching to support patients, identifying needs and setting goals to achieve personalized health and care plan objectives. The role involves meeting patients face-to-face, on the phone, or via video call in GP practices, and working in the community at pop-up sessions to offer self-management education on healthy lifestyles. Ongoing supervision and support will be provided to develop skills and experience.
The role is based at Fryston Road Surgery but involves working across all six practices within the PCN.

* Coaching or counselling qualification/experience or relevant experience involving reflective listening skills; training in non-clinical SSM health coaching preferred.
Experience
* Experience with coaching approaches/frameworks, motivational interviewing, or similar helping strategies.
* Experience working with patients on nutrition and health.
* Supporting patients, families, and carers in related roles.
* Delivering group programs or support.
* Working with the VCSE sector, including volunteers and community groups.

This role is subject to a DBS check due to the Rehabilitation of Offenders Act (Exceptions Order) 1975.
Salary: £28,300 to £31,000 per year, depending on experience and skills.
