Community Occupational Therapist - Adult Sociallady Care
Location: Bexhill, East Sussex | Contract Type: Full‑time | Pay Rate: Up to £32 per hour | Contract Duration: Ongoing | HCPC Registration Required

This is an exciting opportunity for an OT seeking a community‑based role that focuses on promoting independence, rehabilitation, and quality of life for adults living in their own homes or supported settings.
Key Responsibilities
* Conduct comprehensive functional assessments of patients in their home or community environments.
* Develop and implement personalised rehabilitation plans, focusing on mobility, self‑care and daily living skills.
* Provide equipment and adaptations recommendations to support safe and independent living.
* Work as part of a multidisciplinary team (MDT) including physiotherapists, social workers and community nurses.
* Support hospital discharge planning and facilitate safe transitions into community settings.
* Monitor and evaluate patient progress, adjusting care plans as required.
* Maintain accurate and timely patient documentation in accordance with professional standards.
* Contribute to service development, supporting the delivery of high‑quality occupational therapy interventions.
* Educate patients, carers and family members on rehabilitation techniques and safe equipment use.
* Participate in case discussions, clinical supervision and continuous professional development (CPD) activities.
Requirements
* HCPC registration as a qualified Occupational Therapist.
* Degree or diploma in Occupational Therapy.
* Proven experience in community or rehabilitation settings.

* Excellent communication and interpersonal skills with patients and multidisciplinary colleagues.
* Strong assessment787, documentation and time‑management skills.
* Must be a registered car driver with access to a vehicle, as community travel is required.
