To be responsible for the delivery of practice nursing services, working as part of the practice's multidisciplinary team to provide care within their scope of practice to the entitled patient population. The practice nurse will be responsible for clinical areas such as health promotion, chronic disease management, health prevention, well woman and well man clinics, and supporting the management team in reviewing clinical policy and procedures.

Job responsibilities
The core responsibilities of the practice nurse include, but are not limited to:
* Developing, implementing, and embedding health promotion and well-being programmes
* Implementing and evaluating individual treatment plans for chronic disease patients
* Supporting patients at risk of long-term conditions to prevent adverse health effects
* Providing routine nursing care in accordance with clinical evidence, NICE, and NSF guidelines
* Providing wound care (ulcers, Doppler, etc.)
* Requesting and processing pathology services
* Delivering chronic disease clinics and referring patients to secondary/specialist care as needed
* Maintaining accurate clinical records and ensuring effective use of read codes
* Maintaining chronic disease registers
* Developing and embedding well woman and well man clinics
* Chaperoning patients as necessary
* Assisting GPs with minor surgery
* Prioritising health issues and intervening appropriately
* Supporting the team in clinical emergencies
* Recognising, assessing, and referring patients with mental health needs
* Implementing vaccination programmes for adults and children
* Supporting patients with prescribed or over-the-counter medicines within scope
* Liaising with external services to support vulnerable patients
* Delegating clinical responsibilities appropriately within scope and ensuring safety
* Supporting safeguarding matters as per policies
* Understanding and referring substance abuse and addictive behaviour issues
* Delivering opportunistic health promotion
Secondary responsibilities may include:
* Leading audits

Qualifications
* Registered Nurse
* Postgraduate diploma or degree in Primary Care
Experience
* Primary care environment experience
* Experience in chronic disease management
* Experience as a practice or community nurse
Clinical knowledge
* Wound care, sutures, staples
* ECGs, venepuncture
* New patient medicals, chaperone procedures
* Requesting and processing pathology tests
* Chronic conditions (diabetes, asthma, CHD)
