Job Summary
We are looking for a compassionate, motivated Assistant Practitioner – Pathway Facilitator to join our evolving Community Place‑based team – Frailty Service.
The role of a Pathway Facilitator will provide you with the exciting opportunity to work in a rewarding role, where the focus will be on delivering high‑quality holistic care that keeps the Walsall population safe, well and supported within their own homes.

Key Responsibilities
* Support the needs of patients in their own homes within 24 hours of hospital discharge, including observations, medication checks and skin checks.
* Assist with short‑term and long‑term care needs to prevent unnecessary re‑admissions to hospital.
* Communicate effectively at all levels of the organisation to a variety of health professionals, users and carers.
* Provide the interface between hospital and Primary, Community & Social Care settings.
* Challenge systems, professionals and decisions, and effectively communicate the rationale for such challenges.
* Keep accurate contemporaneous documentation and care plans.
* Use electronic alerts/databases (CANAR, whiteboards) to assist with communication.
* Collaborate closely with the high intensity user team and assess risks to patients that need to be managed in relation to their health and well‑being.
* Support the reduction in avoidable hospital admissions.
Person Specification
Essential
* NVQ Level 3 in Health and Social Care or equivalent qualification and/or relevant experience.
* Completion of Foundation Degree in Health and Social Care.
* Current experience working within a health/social care setting.
* Ability to organise own workload and use own initiative.
* Ability to work effectively as part of a team.
* Ability to utilise relevant IT software (e.g., Microsoft Word, e‑mail).
* Effective communication and interpersonal skills with a proven ability to work with other professions.
* Effective time‑management skills.
Desirable
* Ability to work within the lower level competencies as developed within the community locality teams.
* Awareness of local clinical policies and procedures.
