Position: Nurse Case Manager
Salary: £45,000 per annum
Location: Wimbledon Head Office Base
Working Hours: Monday to Friday, 9am to 5pm

Role Responsibilities:
As a Nurse Case Manager (Tracheostomy Experienced), you will provide face-to-face support to clients and their families, develop personalized care plans, and train/support carers with clinical skills and activities.

Key Requirements:
* Registered Nurse with at least 12 months' UK experience and active NMC PIN
* Tracheostomy and ventilator experience (desirable)
* ITU/HDU experience (desirable)
* Knowledge of spinal patient care (desirable)
* Flexibility and responsiveness to changing priorities
* Strong relationship-building skills with clients, families, and carers
* Supportive management style
* Confident, passionate, with a "can-do" attitude
* Ability to manage community caseloads both in community and at Wimbledon head office
