Role: Senior Care Coordinator - Domiciliary
Location: Essex (Chelmsford)
Hours: Full time ( Monday – Friday, 8:30 AM – 5:30 PM )
Salary : £28,000 - £30,000 per annum

Job Purpose
To oversee and coordinate the provision of high-quality, person-centred care to clients in their own homes. The Senior Care Coordinator is responsible for assessing client needs, developing and reviewing individualized care plans, managing care staff schedules, and ensuring compliance with regulatory standards such as those set by the Care Quality Commission (CQC). This role acts as a key liaison between clients, families, healthcare professionals, and care teams to promote client wellbeing, independence, and satisfaction. Additionally, the Senior Care Coordinator supports and mentors junior care staff to maintain excellence in service delivery.
Key Responsibilities
1. Client Assessment and Care Planning
1. Conduct comprehensive assessments of clients\' physical, emotional, social, and psychological needs.
2. Develop individualized care plans in collaboration with clients, families, and healthcare professionals.
3. Review and update care plans regularly based on changing needs.
2. Coordination of Care Services
1. Coordinate with healthcare providers, social workers, caregivers, and service agencies to ensure continuity of care.
2. Arrange for medical appointments, home health services, rehabilitation, or assisted living placements if needed.
3. Monitor service delivery to ensure quality and adherence to care plans.
3. Communication and Advocacy
1. Act as a liaison between clients, families, and care teams.
2. Educate clients and families about available services and their rights.
3. Advocate for clients\' needs within the healthcare and social services systems.
4. Supervision and Leadership
1. Oversee and mentor junior care coordinators or case managers.
2. Train new staff on care coordination processes and organizational protocols.
3. Provide support in complex or crisis situations.
5. Administrative and Reporting Duties
1. Maintain accurate documentation of assessments, care plans, communications, and follow-ups.
2. Compile reports and updates for management or external agencies.
3. Ensure compliance with organizational policies and regulatory requirements.
6. Risk Management and Crisis Intervention
1. Identify risks related to client safety or mental health.
2. Implement preventive strategies or emergency response plans.
3. Coordinate hospitalizations or emergency services if required.
Essential Criteria
* Experience: Minimum 3-4 years’ experience in senior care role or Coordinator role within domiciliary care, demonstrating proven operational and team leadership.
* Qualifications: Possession of a Level 2 or 3 in Health & Social Care (or equivalent) is essential. Please note- Candidates working towards this qualification will also be considered.
* Regulatory Expertise: In-depth knowledge of CQC standards, the Health and Social Care Act 2008, and other relevant statutory regulations.
* Leadership & Communication: Exceptional leadership skills with the ability to motivate teams, manage complex challenges, and communicate effectively with diverse stakeholders.
* Operational Competence: Strong IT literacy with excellent organisational and administrative skills, ensuring efficient management of day-to-day operations.
